Having a fresh new head of glossy and healthy hair following a hair transplant isn’t something you can usually keep quiet about – just ask Wayne Rooney.

However, one clinic has launched a new undetectable hair transplant procedure and has reported an increase in the number of patients keen to keep their implants under wraps.

Some, they say, are even hiding it from their nearest and dearest.

Hair transplant specialist Dr Kouremada-Zioga, who offers the ‘undetectable’ U-FUE hair transplant technique, says that the vast majority of her patients like to keep their procedure as quiet as possible – and as many as one in 10 even hide it from their wives.

‘Very few doctors worldwide are able to perform the unshaven FUE procedure because of the levels of skill involved, and I have patients travelling to London from all over the world specifically to receive this form of undetectable hair transplant which is easier for them to keep quiet about.’

She explains that the single hair follicles are extracted with the hair still at its normal length, but then are cut and implanted as just the root.

‘Therefore, when a patient leaves the clinic, their hair looks the same as when they entered, apart from some initial redness, which patients can cover up or disguise with their existing hair until it subsides over the next few days,’ she adds.

The growth of the newly transplanted hair will then occur slowly and naturally over the coming 12 months and because the change is gradual, the clinic claim that few people notice the difference at first.

Dr Kouremada-Zioga adds: ‘Most of the patients who tell me they aren’t telling their wives about treatment are travelling to London from overseas.

Translating Our Findings Into Autologous Cell Therapies

RepliCel’s current research activity is focused on the company’s core understanding of the unique biological function of hair follicle cells. Two distinct autologous cell therapies are currently in development. The first is a cellular treatment for androgenetic alopecia (RCH-01) while the other is for the treatment of chronic tendon injuries (RCT-01).

RCH-01 – Treatment for Pattern Baldness

The product development path of RCH-01 effectively began in 2000/03 when RepliCel’s founding scientists; Drs. McElwee and Hoffmann; began focusing on dermal sheath cup cells (DSCC). Together they hypothesized that these DSCC were a reservoir of cells that were responsible for the continued health of the hair follicle and the normal cycling of the hair fibre. They believed that if these DSCC were in deficit due to sensitivity to androgen hormones (the cause of pattern baldness), then isolating these same cells from a patient’s own scalp in an area where the cells are unaffected by androgen and moving them to the affected area would resolve the cellular deficit and rejuvenate the hair fibre producing cycle. Multiple experiments on mice demonstrated that hair follicle DSCC could induce new follicular growth as well as cause resident hair follicles to grow thicker and longer. The scientists’ landmark study was published in the peer-reviewed Journal of Investigative Dermatology© in 2003. Together, the scientists filed patent applications that, to date, have been issued in Europe, Australia, and the US, with additional patents pending in the European Union, Canada, Japan and the United States.
These results have led the company to believe in the effectiveness of the procedure and its potential to become a solution to hair loss for those suffering with pattern baldness.

RCT-01 – Treatment for Tendinosis

The product development path of RCT-01 effectively began in 2008 when RepliCel’s collaborator, Dr. David Connell, began focusing on skin-derived fibroblasts as a treatment for tendinosis. Dr. Connell hypothesized that the main underlying reason of chronic tendinosis was a deficit of tenocytes (fibroblasts) in the tendon. As these fibroblasts are responsible for producing type 1 collagen, the primary collagen in human tendon, it was theorized that injection of collagen-producing fibroblasts into the injury site could initiate normalized healing of the tendon. Dr. Connell conducted three Phase I clinical trials using this approach producing evidence that treatment of tendinosis with autologous expanded skin-derived fibroblasts was both safe and effective and should be explored in larger human trials. In 2011, RepliCel began collaborating with Dr. Connell on the development of this technology. Based on the knowledge that fibroblasts from the dermal sheath of a hair follicle can produce upwards of five times the amount of type 1 collagen than skin-derived fibroblasts. RepliCel has expanded on Dr.Connell’s research with the intention of conducting a phase II clinical trial in 2013.

Dr. David Connell has already conducted three phase I human pilot clinical trials focusing on each of Achilles, patellar and lateral elbow tendinosis (tennis elbow) using skin-derived fibroblasts. A total of 104 tendons were treated using these fibroblast cells and no adverse events related to the cell therapy were reported in any of the completed trials. RepliCel intends to initiate Phase II trials in all three indications beginning with Achilles tendinosis using its autologous cell product, RCT-01.

Our Procedure

How It Works

RepliCel’s hair regeneration procedure is based on a patented method for efficiently isolating human dermal sheath cup cells from a subject’s healthy hair follicles and preparing them for re-injection to the donor subject. These cells are isolated from a small skin biopsy taken from an area on the back of a subject’s scalp that still grows healthy hair follicles and are replicated in culture for injection back into the subject.

Once these cells are replicated into the millions in a proprietary culture medium, they are re-injected back into areas of hair loss on a subject’s scalp. It is expected that human scalp areas treated using RepliCel’s process will show increased hair count and density.

About Hair Regrowth And Dermal Sheath Cup (DSC) Cells

The hair follicle is composed of several different cell types. Dermal sheath cup (DSC) cells are located at the lower pole of the hair bulb in a position which surrounds the hair bulb in a ‘cup-like’ manner. These cells have been found to retain regenerative and hair growth inducing capacities.

RepliCel’s pre-clinical studies have demonstrated that cultured DSC cells are capable of directly reconstituting the dermal papilla (DP) and promoting the development of mature hair follicles. During these studies, DSC cells migrated into pre-existing hair papilla and also formed completely new hair follicles.

In RepliCel’s studies comparing cells from different areas of the hair follicle, DSC cells induced better quality hair growth than cells of the DP. Research has also shown that hair growth in regions injected with DSC cells is more evenly distributed and with the appropriate hair growth angle relative to regions injected with other hair follicle cells.

RepliCel has developed a method for isolating DSC cells. The cells are cultivated and propagated in proprietary cell culture using a special nutrient medium to increase cell numbers.

Once these cells are injected into areas of hair loss in humans, our scientists anticipate they will develop hair-producing follicles and initiate natural hair regeneration. The cells are expected to also migrate into follicles that are still present but in early stages of damage – this is a rejuvenation measure and also a preventative measure against further miniaturization of the hair follicle. The new hair follicles and rejuvenated hair follicles are expected to have resistance to miniaturization caused by androgen hormones, the major cause of androgenetic alopecia.

The new cells injected are from androgen-insensitive scalp sites and as a result are less likely to be affected by these hormones. In rejuvenated follicles some original androgen-responsive cells will remain. However, the injected cells will not respond to androgens. The growth signals from the injected cells should compensate for the loss of signal from the resident androgen-responsive cells.

RepliCel’s Phase I clinical trial conducted at a clinical skin center (Scientific Research Institute for Skin and Venereal Diseases) in Tblisi, Georgia, determined that the RepliCel procedure is safe and effective. RCH-01 – Hair Regeneration

Phase II – Treatment for Pattern Baldness

Based on the promising results from the phase I/IIa clinical trial, the RepliCel team is currently preparing a formal clinical trial application to conduct an in-depth phase II clinical trial with RCH-01 in Germany. Guidance received at meetings with the Paul-Ehrlich-Institut (PEI), the German competent authority for human clinical trials involving cell therapy, has allowed the team to design a robust clinical trial to confirm the safety and efficacy of injections of RCH-01 in approximately 120 male patients with pattern hair loss.

The RepliCel team is also pleased to be partnering with one of the largest hospitals in Europe, the Charité-Universitätsmedizin in Berlin, as the primary clinical site for this trial. The Company anticipates initiation of its phase II study in the second half of 2013.

RepliCel’s Phase II study will be supported by a new cell culture protocol. This new process has given rise to improved cell replication and cell vitality leading to a more robust cellular product available for injection. This second generation process has been reviewed by the German regulators and is consistent with our goal of isolating the lowest effective dose to maximize hair growth.

RCT-01 – Tendon Repair

Phase II – Treatment for Achilles Tendinosis

RepliCel has initiated the clinical development of a new autologous cell-based treatment for common tendinosis caused by acute and chronic tensile overuse. This technology is based on the company’s core knowledge of the cell structure of the hair follicle and the biological activities of those cells. Phase I clinical data from our collaborator Dr. David Connell – indicates that the engraftment of collagen-producing skin-derived fibroblasts is safe and effective at repairing micro-tears in the tendon. The injected fibroblasts are thought to promote the regeneration of tendon tissue that has been damaged by chronic Achilles, patella or lateral elbow tendinosis.

The concept of using skin-derived fibroblasts to treat chronic tendinopathies has been tested in three pilot trials. In one of the trials that focused on the Achilles tendon, 24 patients aged 22 to 67 years old  (mean age of 45.2 years) with unilateral tendinosis were randomized into treatment (12) and control (12) groups. The patients in the treatment group received skin-derived fibroblasts suspended in autologous plasma under ultrasound guidance. The control group only received a local anesthetic. The treated group had statistically significant (p<0.001) improvements over the control group in both function and pain thus demonstrating the treatment to be both safe and effective.

The first in man trials including patients with patellar tendinosis and lateral elbow tendinosis (tennis elbow) both demonstrated that the treatments were safe. In the patellar tendon trial, there was a statistically significant (p<0.001) improvement in the treated group. In the lateral elbow trial there were not enough patients to power significance; however of the 12 patients, 11 had a satisfactory outcome.

The above phase I studies tested fibroblast cells isolated from a patient’s own adipose tissue. RepliCel has isolated a more robust source of fibroblasts from the dermal sheath of the hair follicle. These non-bulbar dermal sheath (NBDS) cells are much more prolific in their production of Type 1 collagen, the major cell structure of tendon, and are easily harvested from hair follicles taken from a micro-biopsy from the back of a patient’s scalp. These cells are then isolated and replicated under cGMP conditions for re-injection into the site of the tendinosis.

In chronic tendinosis, a key impediment to healing is the lack of available tenocytes (fibroblasts) to initiate the production of collagen used in the repair of damaged tendons. While Dr.Connell’s work successfully focused on addressing the cellular deficit through use of skin-derived fibroblasts, RepliCel’s collaborative effort will focus on the use of more robust NBDS cells in a proposed phase II Achilles tendinosis clinical trial, which is anticipated to be initiated by the end of 2013.

RCI-02 – Injector Repair

RepliCel is developing a new dermatology injector device designed to provide an improved level of control and precision for dermatology injections. The device is being developed to support the company’s cell-based hair restoration technology but will also be marketed for other dermal procedures.

The RCI-02 Injector (Gen II) will be able to deliver substances not only into specific layers of the skin, but also to muscle tissues and tendons in a slow and constant form without any pressure or shear stress, ensuring the injected substance is viable and healthy after application. By improving the conditions of substance delivery, we improve the chances of success in the treatment of the subject. Furthermore, the injector is capable of delivering tiny injection volumes, with minimal pressure, and will incorporate cooling at the injection site, thus making procedures less painful. This is a significant improvement over using a standard syringe where the needle depth and angle is shifted each time the injection site is changed.

In addition to providing a means of accurate delivery of the Company’s RCH-01 and RCT-01 products, the Gen II injector will also have significant applications for other dermal treatments including dermal fillers, botulinumtoxin´s or other cell types such as fat cells from liposuction for lipofilling.

Home remedies to treat hair loss

If you are experiencing hair fall, you can control it with inexpensive home remedies. These help in strengthening the roots to initiate new growth. Some of these are:

Onion: Squeeze an onion and take juice out of it. Add one to two teaspoons of honey to it. To do away with the odour, you can use rose water with onions. Mix and apply it on your hair. Keep it for 40 to 50 minutes. Then wash your hair.

Curry leaves: You can make an effective hair tonic by boiling some curry leaves in your hair oil and applying it on your scalp. Massage it well for at least 15-20 minutes. Follow this procedure twice a week to stimulate your hair growth.

Coconut: Coconut provides a host of ingredients that naturally condition and promote growth of hair. The milk of the coconut contains proteins, essential fats and minerals such as potassium and iron; using it regularly can reduce the breakage of hair. Coconut oil also carries the same rich ingredients and it helps to make the hair stronger right from the root, through the shaft to the tip. Regular use of this oil to massage the scalp has a protective action against hair loss. Grate coconut and squeeze out the milk by mixing in a little water. Apply this to the specific area where you notice thinning or balding. Allow to stay overnight and the next morning, rinse off with water.

Amla: Packed with vitamin C and rich in antioxidants, amla is the perfect solution for most hair loss woes. Besides applying to the scalp, it is also helpful to consume it on a regular basis because the vitamin C is good for your body. Crush the amla fruit to extract a juice or buy the amla powder available at an herbal store. Add 2 teaspoons of the juice or the powder into an equal quantity of the juice freshly squeezed from a lime. Mix well, apply to the scalp and leave it to dry; then rinse using warm water. Other treatments

Hair transplant: This is perhaps the most permanent solution for hair loss. It is broadly achieved by removing grafts/follicles from the back of the head which are more resistant to hormonal changes, and placing them in areas of scanty growth or baldness. There are two methods to remove the grafts – Follicular Unit Transplantation (FUT) and Follicular Unit Extraction (FUE). (Read: Which hair transplant method is right for you?)

Hair restoration: This procedure consists of regrowing hair on the scalp without any linear scarring. A local anesthetic is used to numb the area before the procedure and hair follicles are removed from the non-testosterone sensitive zone at the back of the head. After this, the hair follicles are placed on the scalp. On an average, 6,000 to 9,000 hair follicles are placed on the scalp. The hair begins to grow after about three months of the treatment and thickens up between the fifth and eighth months. Patients can see a full growth within 10 months of the procedure. This method of hair restoration does not use cutting objects like scalpels or sutures which cause scarring and takes up to six hours for one sitting. Hair extensions: Hair extensions are a temporary solution for people who have lost hair either made of human hair or synthetic hair fibre supplements. They can make your thin hair look voluminous. Wax, glue, adhesive or silicon rings are used to attach the extensions to hair. If you are using human hair, you need to take care of it by keeping it well-conditioned. On the other hand, synthetic fibres lose shine and colour due to frequent washing. So be careful with them. 

Stem cell treatment: In this process, a small amount of fat is taken from the waist area of the person whose hair is being treated through a mini-liposuction process. This fat, which contains dormant stem cells, is then put in a centrifuge and spun to separate the stem cells from the fat. Once that is done, an activation solution is added to these cells. The cells might have to be multiplied in number depending on the size of the bald area that needs coverage. Once activated, the solution is washed off so that only cells remain. Now, the stem cells are injected into the scalp where hair loss has appeared. One can find some hair growth in about two to four weeks.

In this technique, a thin and transparent membrane containing human hair is applied to the scalp and woven with rest of the hair for a natural look.

Wefts and strand-by-strand procedures: The process involves selecting the hair type and membrane that matches best with the patient so as to give the hair a very natural look and feel. The colour, hair type etc. need to be similar to their existing hair. Next in line is the adhesive used for bonding the membrane to the scalp. Apart from these, there are also medications to control hair fall depending on the type of hair loss you are suffering from.

For male pattern baldness

Finasteride: It is a pill that blocks the action of natural hormones in scalp hair and reduces hair loss. Approved for use in only men with androgenic hair loss, this drug is found to be safe in postmenopausal women.

Dutasteride: It may help in hair loss by blocking the production of or binding of testosterone in the scalp hair follicles. You are not permitted to donate blood before the six-month clearance time after taking this medication.

For patchy hair loss

Small amounts of steroids like triamcinolone may be injected into your affected scalp patches to stimulate hair growth.

Though these are the available options you can choose from if you suffer from hair loss, in order to avoid it, eat a healthy diet and nourish your hair to keep it from falling. Massage using oils like olive, coconut, almond, etc.
